When I bet $5000 on sportsbook, why does the manager come out and stare at me, then writes something down?
I don't use a players card - is he trying to identify me? Are they trying to make me paranoid because I bet using a winning ticket that was purchased by my boyfriend? Do they take photos of bettors when they place a decent sized wage?

He’s not trying to make you paranoid, but the casinos like to keep track of who the big money betters are. He may remember your boyfriend making the bet, but does not recognize you. He’s probably writing down the time, betting window, and a basic description of you so that if someone comes in claiming that their ticket was stolen, they can look you up on the video system.
Of course, that’s not going to happen since it was your boyfriend’s ticket and you had permission to cash it in. You did nothing wrong and have no reason to be worried about anything.
Casino personnel are generally friendly people and will do whatever they can for people making $5,000 bets on a regular basis. Don’t be afraid to say hello to the sportsbook manager, and for God’s sake, use your darn player’s card! You want those comps on your big bets.
